2. Place the scrap crowns, bridgework, silver alloy (no amalgam) and foils in the plastic bag provided; then put the bag inside the container provided and into the box for shipping.

3. Complete the form accompanying your shipment with your information, including to whom the check should be made payable. Place this form inside the shipping box.
